Occasionally I see a PCGS slab with this designation. and no numerical designation. Why? Is there a lower cost for this designation? Any other reason?

Yes, these are usually large bulk submissions often for TV sales. And yes the cost is lower than having individual grades assigned.

Agree. Not much use to it.
But, as far as I'm concerned it still serves to authenticate the coin.
I remember something semi-recently, might be another also.

Gem Unc is basically equal to MS65 or higher. So one grading a raw coin Gem UNC on the 2x2 holder might use the MS 65 column from CW Trends.
