Signed Type 2 Silver Eagle artist print I framed with an ANACS-graded example. I worked the ANACS booth at the Chicago ANA, and got the coin and the print while I was there. My boss at ANACS had a one-off label made for me.

Definitely ICG - they had a patent on the slab, although it was focused on the tamper-evident stress contour, not the slab per se.
the rail at 3 o'clock is broke and looks like an air hole into coin compartment. I pealed back the reverse to see if anything written under the Franklin mint sticker - looks clean. I still like the color and will probably send in with my next modern submission.

When I de-bork my code (SOMEBODY, who shall be nameless overwrote the file for the SampleSlabs form with generic code from my SplashScreen, so the search & dropdowns don't work anymore) (I can fix it easily, the correct code is in the git tree -2 version, just have to pull it down instead of writing replies to posts here).
Anyway, S3e#PCGS-001-3-2 (-3-1 is the famous blank green label).

Good eyes, Dave. I showed the slab to several people and asked them what was wrong with the label. Nobody got it.
wow. 4 pages of everything but a gen.1 NGC slab with black insert. Oh, and a gen.1 PCGS slab with white label. Somebody is holding out.

So you can sell it either as a rare TPG error, or the even rarer Washington Commemorative Half Dollar (see the TPG called it that).
Or you can get a stuffed mouse and a white cane for your exhibit case on "Blind grading of coins".
