PCGS Error Nickel with an Unusual PCGS Insert

I am not sure exactly how to read this insert. The coin number look-up provides the same information.
I haven't seen this before and am hoping a more seasoned collector of errors would shed some light on what the insert
represents/means.

    I don't know why they could not determine the denomination but here is a link to a similar or E1111.
    It stated E1111 is an umbrella number.

    Here's one with a coin number.

    It's the equivalent of an error on a star (misprinted replacement) note in the currency world. Those are neat.

    No, actually more like a holder error on an error item.

    Collector and dealer in obsolete currency. Always buying all obsolete bank notes and scrip.

    TPGs have label typos on errors all the time, lack of proper QC that has continued for years and years.

    I recently submitted an error US banknote to the current market-leading TPG for currency. I accidentally filled in the wrong Friedburg number on the form, and got it back with the incorrect number on the label. Makes you wonder…

    The label clearly indicates that the coin was struck 150% off center.

    This is from the auction description:
    "Coin holds a Pedigree Sticker from the 8th Great Grandson of Thomas Jefferson."

    So the Pedigree line makes sense.
