Just practicing on my eye for Identifying either one! This is MD!# 1999 P Jeff Nickle.

Certainly. The 1916 DDO-001 5c comes in two stages-one with MD and one without. The stage with MD seems to be much the scarcer of the two.

Is there a known DD for 1999P nickels? (I don't know, myself). If not then why look? That's just my opinion.
MD is "flat, shelflike". That seems to describe your coin. Also, the width of the numbers seem to be reduced by the doubling, another sign of MD.
Yes. The 1931-S DDO FS-101 Mercury Dime has Class II Distorted Hub Doubling that is identified on the date but strike doubling can also be identified on the profile. There are many examples like this.

Yes, it's possible. They are caused by 2 different things. However, Your coin is not a true doubled die
