The Obv. die was slightly mis-aligned,
which permitted the sliver ('fin') of metal
to flow into that small thin gap.
Although scarce on Proofs, there are numerous known
examples of both Clad and Silver proofFinned Rim quarters.
Buffalo 1 oz. bullion gold pieces were struck in quantities
with this anomaly.

Seems that someone placing it in the proof set box would notice. Although they may have done it intentionally for fun.
