I've seen plenty, particularly in the 1999 to 2002 era that I didn't think merited the DCAM designation but were in DCAM holders anyway. They lacked what I consider the frost and contrast necessary for the designation.

I have experienced the same. I think, the services pretty much automatically assign DCAM to many moderns without even considering the coin. There are MANY modern coins that are in DCAM holders, that probably wouldn't even CAM if they were, say, a Morgan Dollar with the same amount of frost and field depth.
I've owned a couple NGC PR69 Ikes that were not even CAMEO (thus no designation as such on the insert). They are surprisingly attractive coins. Clad proofs without cameos are pretty. Very watery looking.
And the 64,000 dollar or post question is........ what is the give-a-way? My guess a lock of his hair, some beard trimmings, a pack of smokes full of half smoked butts and a warm can or his beer choosing.
I think I've seen some that I would call "CAM" not "D-CAM" and "ULTRA CAM," but I've never seen one in less than "CAM."
Retired dealer and avid collector of U.S. type coins, 19th century presidential campaign medalets and selected medals. In recent years I have been working on a set of British coins - at least one coin from each king or queen who issued pieces that are collectible. I am also collecting at least one coin for each Roman emperor from Julius Caesar to ... ?

I have three West Point Comm Proofs, graded at PCGS a) PR, b) PR CAM and the third PR DCAM.

The more qualities observed in a coin, the more desirable that coin becomes!
