Was someone messing up quarters, or is this some sort of mint error?

I pulled this from a coin roll. From what I know and have learned reading here and elsewhere, this looks like someone got a little crazy with a grinder of some sort. There are scratches on the obverse and reverse. The reeding on the rim is also largely gone. Am I right in thinking this is PMD, or is it possible that something went very wrong when minting this coin? I just want to be sure before I throw it back.
