old ANACS picture cert with an AU55/55 grade. Coin could be pictured (should be able to tell from picture (sometimes switched)). House in Capital Plastics holder (some have purple velvet pouch).
The reference to SAMPLE is something someone added along it's travels.

It is not a SAMPLE of the photo certificate.
From the wording, it's a grading set type item. "This is a sample coin showing what ANACS is currently grading as AU55".

There are many that collect these.. I know of at least 2 extensive collections/hoards of these...

In my early days I had one with a coin in it. Now it's a plain holder and no coin. Wish it was full.
