Greg Maddux. MLB pitcher of the month award. June 1988, August 1993, and April 2006. He actually has a few more of this particular award but I listed these particular 3 to satisfy the post question.

if I get this one correct; I'm going to take all of my winning cash to rent an empty truck and fill it up with unopened material... :D

that was a good call on Maddux winning pitcher of the month in 3 different decades. I looked on BR and clemens has won a ton of awards etc in 3 different decades: pitcher of month, pitcher of week, WAR for pitchers, ERA, there were more, but you get the point.

George Brett, Roger Clemens and Tommy Brady.

Jean Beliveau was an NHL all-star 13 times in his playing career that spanned from 1953-1971. He was also a Stanley Cup champion 10 times during that span for good measure. Also he has his name etched on the Stanley Cup 17 times in total when you include his time as a Habs executive with the final inscription being in 1993. So in short, his award legacy runs from 1953 to 1993 in the NHL.
