I'm just guessing they'd assume it had unnatural wear at some point to wear off the entirety of the reverse while leaving a decent amount of the obverse (Obv looks like a 2 to me) intact.
You have to remember, as long as you could see the position of the first star, the drapery area, and the 8, you could identify it as a '38 large stars no drapery.

It would very hard to have that much detail on the obverse, but nothing on the reverse naturally. It can happen on a 1794 or ‘95 half cent or large cent, but on a coin from this era.
After seeing a 1796 quarter that was worn smooth except for the date in an NGC holder, I'm more on the optimistic side.

I don't think it will slab as problem-free. It appears to have too much remaining detail on the obverse to have a completely blank reverse unless it was planed or ground at some point.

It would be a waste of money to submit that coin. I believe, as others have said, that the reverse has been intentionally defaced. Cheers, RickO
I wouldn't. The reverse looks like it was smoothed intentionally for a love token but never engraved. Then just took on wear.
