Doctoring or not - ethical or not?

Let's take a coin, doesn't matter what coin, that grades AU details, lightly cleaned. The coin is then carried or circulated until the grade becomes VF30.
Has this coin been doctored?
Does the intent (pocket piece vs. currency use vs. profit motive) factor into your opinion?
Does the relative value of the two grades (cld. AU being worth more or less than straight VF30) factor into your opinion?
If the coin is sold by a dealer who knows the coin's history, are they ethically obligated to disclose that history?
