I believe this is known in coin collecting circles as "the ring of death". It is damage, imparted onto a coin either by a coin wrapping machine or by a vending machine.

What you show in your picture is PRECISELY what a coin wrapping machine would do.

The inner circle on your coin is not raised. It may have a raised edge. Consider what happens when you drag a plow through a field (or a fork through mashed potatoes if you prefer): anything that you scratch up has to go somewhere.

Yes. It raises metal when it digs into the die.

It is not a doubled die. You might want to Google the term.
