Couldn't say slightly off center with thus one. First I've had with full date and mm this far off. Guessing 90-95% off center.

At least it's a full date. Great pick-up.
I have one similar with even less struck, I think it's a 1996-D. odd that the date gets struck but I'd figure it's because of how the die(s) is locked into the press in relation to the planchet feed.
