The staples are AU [although I see PMD on them]
and the quality of the Black ink is VF/XF
the #4 is the total number of grades assigned

The MS is incorrect and misused because the coin obviously looks polished, another thing he or she (likely a YN) will learn someday. But the grade notation is likely; obverse is VF to EF /slash and the reverse is AU. The #4 is likely a notation for the number of coins graded for a date.
