Can anyone explain this on a 2004 D?

    Looks to me like it was rolledthrough the machine more than 3 times.
    Not only that there are other letters visable.

    Damaged.

    Notice how the letters are mushy and the rim is folded over the letters. That's a characteristic of a dryer coin (you can look it up in www.error-ref.com). It couldn't have gone through the upset mill multiple times after it was struck, plus the upset mill is set for a specific diameter, so even if it was possible, the diameter would not get smaller each time.

    Lots of magnified pics, but no pic of the entire front and rev.

    Actually, your close-up pics are very good, but a good pic of the entire ob/reverse would be nice, too.

    Usually people ask for better/closer pics but in this case I'll bet a shot of the entire coin will tell the story.

    That pic confirms It's a dryer coin. It's PMD, not an error
