I see ones like this and I am not sure if it's a mark that has been punched deeper or if it's just some sort of wear. any thoughts? and thanks

Strike doubled.
Yes, it's strike doubling. Sometimes referred to as mechanical doubling. Notice how the doubled area is flat. That's a characteristic that can help you recognize it. It is not considered an error and adds no extra value
