In my opinion
Are you asking about coins that are rotated in their holders or coins that are not lying perfectly flat?
If the former, sometimes coins in slabs rotate from being jostled. Often they can be reoriented properly. (I use a sonic toothbrush to vibrate the slab and turn the coin. I made a video and shared it here).
If the latter, it is usually possible to rap the slab edge on a hard surface to reposition it. Sometimes it's a lost cause and the coin simply won't budge. Beware: rapping too hard can cause stress lines in the plastic or worse.
If you're talking about something completely different you should elaborate so we get it. A picture would be helpful too.

It sounds as if the issue is a rotated coin.....but without pictures, I cannot tell.... Please clarify your problem... a picture would help... Cheers, RickO

Not common.
Sometimes it occurs when slab is dropped or something dropped on it or it could occur because of vibrations during travel.
Fixing it is the issue. A rotated coin could be fixed with a vibrating toothbrush applied to the edges. Sometimes whacking the edges will work, too.
If the coin popped out of the prongs or whatever is holding the coin it might need to be re-slabbed.
