  Not an error, the coin is damaged.

  We'll never know why someone did this to the quarter,
  but as others have said, what you see was done to the
  coin after it left the Mint.

  Interesting that the 'indented rim damage' isn't uniform
  as far as distance apart.

  I remember, somewhat, that a similar tool was used to make the indents around the rim; but it was somewhat concave.
