The coin is in a Treasury Hoard Silver Dollar slab. I'm not familiar with these at all. What going on with the spikes in the lips and chin ? There's a spike behind the head also ?

usually you can see the corresponding clashing from the obverse on the reverse. plenty of exceptions and depending on strength of clashing.
