David Schwager lists this as S# NGC-010-3-9. From the 1st edition:

This is another uncommon sample that camouflaged itself as a common piece. Only four 199999 proof diles appeared on eBay during the survey, but prices were only $15-$30. All had 1961 dates and single digit serial numbers.

At -012 this is higher than was previously seen.

Thanks very much, so I'm looking at $30? Or is it rarer than I thought?

That was David's best guess. With thinly traded material and a limited set of collectors, prices are hard to estimate. Last month or two several common sample slabs have languished w/o a bid, while a few unusual pieces have traded strongly.
