From what I can tell, it actually looks like a large portion that came between the dies was actually ripped or folded off post strike. I could be wrong.
This one looks like a generic off center which had the struck portion come detached after the strike. Occasionally errors like this one are damaged by counting or rolling equipment, I think the edge of the clip will show signs of trauma consistent with PMD.
