Neat design! It looks flat, though --- like the design is raised above the
lowest field of the coin, but is flat.

This. I'd call it "extruded relief." Perhaps a high relief design was beyond the capability or budget of the manufacturer.

Is this a US Coin or has it ever circulated in the US as such? I could not find it in the Red Book or on the US Mint Website.

Maybe the folks over on the Precious Metals Forum or World & Ancient Coins Forum would know something about it?
