Die letter gets filled with junk, which eventually falls out onto a planchet, as it is being struck. The junk which is shaped like the letter causes an incuse letter to be struck on the coin.

There might be a slight perspective which might confuse exactly what you are seeing here, the dropped letter is shifted around and now the top of the dropped letter was originally the bottom loop of the "S" (dot follows bottom loop)
