No surprise that Wes Ferrell had a couple as a starting pitcher. Earl Wilson hit one in 1966, but as a pinch hitter. Couldn't find anything else. I'll keep looking
I remember getting my Bowman Chrome Soriano graded and getting a 10 on it as well. This was in like 2002 and I believe I received well over a hundred bucks for it and I kicked myself for selling it. I thought it had alot of upside! LOL

Collector has the correct answer, which IS Nick Swisher.
He hit a walk off homer in 2008 (see here: http://ballhype.com/article/nick_swisher_home_run_sends_white_sox_to_dramatic/)
and he appeared as a relief pitcher for the Yanks this year: http://www.baseball-reference.com/players/s/swishni01.shtml
Your question does not indicate that it need be a full time relief pitcher, or that the home run be hit in the same game as the player was pitching
