It has happened very, very rarely, and when it has been detected PCGS has stepped up to the plate and made good on them. Ditto for NGC and probably ANACS. I image that some of the second string slab companies pay too, but when it comes to the bottom feaders, and we know who they are, it's YOUR problem.

Yup. Seen a few Omega 10s & 20s, couple low grade CBHs, a Seated or 2, couple fake coppers, a 20th cen key or 2. Slab companies made good on the coppers & 20th keys as far as I know. The gold & silver type bring good money from advanced collectors. Tried to buy a slabbed Omega HR, it brought more than a real one. Shoulda bought it, cool coin, funny in the holder.

Razor, yes there are counterfeit PCGS slabs, at least of the first generation holders. They hit the market back in 1988. The person producing them was arrested and spent time in jail. PCGS offered to extend their guarantee to cover them as well. Most of them are off the market and I would dearly love to get my hands on one for my reference collection. I'm not aware of any other generations of PCGS slabs being counterfeited.
There have also been coins slabbed in counterfeit ACG, SEGS, and ICG shells as well but in none of those cases were the slabs marked so as to try and actually pass them off as being ACG, SEGS, or ICG slabs. (Counterfeit shells but with a different company name on the label.) I have heard of counterfeit NGC slabs put have no confirmation of this.
