Yes, I have one pictured in the slab book. It's a Washington Born VA cent that has right on the label Damaged or Tooled. Interestingly, it is NOT graded.
I have personally seen and handled 2 colonials that were in PCGS holders, graded as normal (meaning apparently not net graded), that had been sold previously in auctions raw described as 'holed and repaired'.
Of course that could mean that the previous cataloger was wrong.
Yes. I've seen PCGS coins with initials in the field. They were Bust Dollars. I don't know if they missed or not. The dealer claimed they are "net graded."
