Can't say I've seen one that... flat.
Is it the same size as a normal large-size 10p coin? I'm wondering if it might have been railway-tracks-flattened, as well as whatever other history it might have experienced.

I doubt that's purely circulation wear. Pre-decimal coins used to get like that but they could circulate for over 100 years. This circulated 25 years at most.
Note the rims - something has worn those off.
