He's saying that MD looks different (like your coin) on a coin with incuse lettering. I don't know exactly how to describe it. On normal lettering, the doubling is on the face of the coin at the bottom of the letters so you get that very low, flat shelf-like phantom image. For incuse lettering, the letters are inverted and the face of the coin is at the top of the letter resulting in an "inverted" shallow shelf-like image that looks like it is deeper than it is because the shallow shelf is at the very top of the channel.
