Hmmmm... What exactly would this error be called?

What's the correct name of this error on this coin? Yes I know it has a nasty carbon spot but it's an error none the less. It was offered to me from a customer and I couldn't pass it up for inventory.

I would rearrange that a bit placing more emphasis on the D/S as in...

Double struck with rotation, second strike 5% off center.
