Did this happen at the Mint or is it PMD?

I found this one in a quarter roll. It looks like someone might have hit this with a punch of some sort (although why they would I can't guess) The odd thing is that there is some sort of fairly clear partial character in the impression left. Could this be a double strike of some kind at the mint, or is it just someone playing with sharp objects?
