Quarterly Special Question

I am getting ready to submit some modern coins for entombing and saw that the Quarterly Special is a nice slab holder that would look good on my desk. The instructions for obtaining the QS state "Under the “Other” service level, indicate Coin Stand QS" and it sounds like I need to use a printed form. I fear the printed form as despite the nun's efforts in my Catholic school education my handwriting can still be described as chicken scratch. Is there not a way to get a QS via the on-line form?

I did not go into the online submission form to verify, but this page indicates the QS requires a PDF.

PCGS Coin Stand Quarterly Special Guidelines:

Minimum 5 coin submission to qualify.
Limit one per submission.
Valid for unlimited submissions while supplies last.
This offer is available while supplies last.
Available for all PCGS service levels excluding all Show levels and Bulk.
Valid for U.S. or World (non-U.S.) coins. (Cannot mix)
Under the “Other” service level, indicate Coin Stand QS.
Additional Guidelines:

All submissions must be submitted on a PCGS Submission Form
For PDF Submission Forms please visit PCGS.com/forms
All submissions must be postmarked by Friday, March 31st 2023
Turnaround time is estimated and is based on the service level selected.
Standard shipping and handling fees apply.
Calculate return shipping and insurance fee based on the total quantity and value of all coins on each submission.
Valid only for submissions to PCGS U.S. headquarters.

Never used it myself, but I think that you can populate this form and then print it out.
