The lettering is so soft I thought perhaps it was minted from grease-filled (partially) dies.

Not normal circulation. I have seen half dollars like that and the rims are flat. Maybe it went through some machine.

Probably squeezed between two semi-solid objects like wood.
Looks like excessive wear.... long time in a pocket with other coins... or some similar situation. Cheers, RickO
