Deformed Lincoln Cent - What happened? (1961-D)

I found this deformed Lincoln Cent 1961-D with some other pre-1981 copper cents in a Coinstar machine:

Normally these coins are 19 mm wide but this one is 22 mm wide.

I think the technical term is "Squished". The right side of the coin may have ended up in some type of press or maybe something else. Whatever caused it, it's not an error

If you get a deformation that is dispersed evenly in all directions, rather than just one area and direction like your coin, you can end up with a coin like this (the one on the left is un-deformed normal size):
