interesting nickel - error or pmd?

I got this shield nickel with a group and am trying to decide what it is and what could have possibly made it look like it does today. Got any ideas?

    Looks like a clumsily filled hole

    Not so fast speed racer. Looks like there may be some die striking within that 'hole'. This might be more interesting than PMD.

    Close-up photos are nice, but also show both sides of the whole coin.

    That will always help in diagnostics.

    Pretty cool. To me, looks like a puzzle piece sunk downward from its original position. :*

    If you squeeze the arrows together, the piece would be back in place.

    How it was formed is the question? PMD? Probably. :)

    PMD... As to the how or why? I have no clue.... but definitely not of mint origin. Cheers, RickO
