It was probably ground down to be used as a quarter in a vending machine. Similar to what had been done with cents to be ground down to be used as dimes in a payphone back in the day.

In general, when looking at possible error coins, just ask yourself if there is any way you could make that yourself with tools. If the answer is yes, then it is not an error.
Definitely PMD.... There are videos available of the coin minting process, as well as literature.... study that a bit and you will understand more about what constitutes a mint error. Cheers, RickO
