Per the PSA web-site, these are the officially recognized grading qualifiers:
OC (Off Center), ST (Staining), PD (Print Defect), OF (Out of Focus, can honestly say I have never seen or heard of this qualifier until today), MK (Mark), and MC (Mis Cut).
If you were part of that meeting, what would you have added (either serious or humorous). Here are mine:
PH (Pin Hole): I've seen way too many PSA 1's that are actually PSA 6's or 7's with a Pin Hole, so maybe they could be PSA 6 PH.
PL (Paper Loss): Same as above.
BS (Bicycle Spokes): Reserved for a card that was clearly used to make motorcycle sounds in a bicycle at some point in its existence.
WM (Washing Machine): Reserved for a card that was in a pants pocket while said pants were washed.

Those are pretty funny. I could add (AA) for artistic addition. Those would be the cards you find in old childhood collections that have an extra mustache or some other addition drawn on by the original owner.

WR (wrinkle): for all of those cards that look like a 9 but have a wrinkle that bump them to a 5.

buying O-Pee-Chee (OPC) baseball
I'm not sure if they still even use the OF qualifier anymore. I sent in a Ryan rookie about 20 years ago that came back as OF. One of the only ones I have seen. I have sent other cards I know that have had focus issues and not asked for "no qualifiers" and they were just bumped down what I believe were two grades.

Honestly I would have said qualifiers are unnecessary. The numerical grade we give will reflect all defects of the card. Instead I would have devised a scale for how many grades are typically deducted for the various defects.
SM - Smells. I'm not talking about the desirable cardboard smell, I mean the funky odors such as damp basement or cigarette smoke.
