From the title of the post I initially thought it may be a state parks quarter struck over a struck (Franklin) Roosevelt dime. Oops! Didn't realize it was ol' Theo.
Struck on an elliptical clip as previously mentioned. Wonder how many of these will end up on the market - quite a few of this particular design have sold. Though, this one is particularly interesting.
I bet a complete set of national parks quarters struck on elliptical clips could be made, and would assume a couple people have already.

Agree. It is possible to build a 2016 or 2017 year set. I myself picked up one on 2017 Effigy Mounds (just as an example, no intention to build set though). They surely are very rare.

Cool! I don't think I have seen a newer error coin with one of the commemorative quarter designs being struck on the wrong planchet before.
