Are grades that get a + as rare as it appears? Do they happen at all grades? Would there ever be a VG8+? Do you think Dr. Sheldon even considered the + grade being applied? If you have a choice between a straight grade but for a little more there is a + one at the same grade, would you go for it? Do you consider a + the same as a CAC bean?
Post the + graded ones you have. Lets test the spread from low to high.
Here is one I recently got while filling holes in my Registry Set.

I have a PCGS MS64+ 1916-D Liberty Walker (Pop 596/21 in 64/64+) that did not receive a CAC bean (Pop 72 PCGS & NGC in 64). Of course, no reason given as to why not!

My only + coin from Angel Dee's, the coin has such an amazing amount of blazing luster that even this pro photo (taken by Mark Goodman) cannot capture it fully.

@ElmerFusterpuck - A hansom large cent if ever there was one. Very good color and details. It deserves the + grade.
@Walkerfan - Hold my beer while I get my sunglasses. Blazing white defined. These would dazzle anyone.
I honestly wonder what the difference is between an AU55+ and an AU58 coin?

@braddick - You will never discover the reasoning. "They" would probably say it's the experienced eye of the grader concluded after a 10 second deliberation it deserved a 55+ and not a 58.
@TNort17 - That has to be one of the best looking Walking Liberty's I have seen. Jaw dropping speechless.

Think I only have a couple. And of those, I'm not sure I'm a good enough grader to split those hairs.
To me, it's a "whatever". This one looks like a 64 to me....and my 62+ Morgan looks like a 63. Flip a coin.
Heres my two. The 50-s is a 67+ and 54 is a 66+. For sale if anybody wants them.
