How in the world is this in a 67 holder?

I'm assuming that is a hit on the cheek, what else could it be?

i could be wrong but i judge the marks in the hair behind the temple as "on the Holder" and seeing how everything is missing in the un-enlarged holder picture leads me to believe all those marks are lighting anomalies caused by marks on the holder. the same with everything going on in the field behind the portrait.
Of course the focus is bad, and the white balance is off too. This is one of those "take-a-chance" buys.
