Which PCGS holder has the most accurately graded coins?
Old Green Holder? - Supposedly not only has an accurately graded coin by what many feel were graded against traditional grading standards, it is often holds a under graded coin desirable for crack out and upgrade.
The Blue Holder ? - More accurately graded as two out of three graders had to agree on the grade. Most agree Blue Holder coins eliminated the under graded phenomena of the OGH. Little wiggle room, if any for upgrades. Some argue coins are more technically graded thereby are not graded accurately.
The Plus Holder ? – No doubt the first coins in Plus holders are often a regraded coin in a former Blue or Green holder. But this will change as more coins are graded to show which coin is high end for the grading scale.
Any of the above with a Bean on it? - What can I say; a coin possibly graded as many as three times. But maybe only twice and confirms the grade is accurate. That means as many as 4 people looked at it.

Then I guess the question becomes, "Which holders have the fewest clerical errors?"
Keeper of the VAM Catalog • Professional Coin Imaging • Prime Number Set • World Coins in Early America • British Trade Dollars • Variety Attribution
What floats around now and rattles has little potential , less it be fresh from being held for 20+ years
oh , I see you were asking about accurate ; that prolly is a loaded question as most the different holders hold coins graded by different people .
