my first thought upon seeing the coin was that it looked like it got treated just like Pacino did at the end of "Scarface" but after reading your post, I see yours is the better analogy

I like the "Tin Foil" comment as tin foil is very soft and malleable and Nickel is a very hard metal

The coin in the OP looks like a legitimate error. There is a similar triple-struck 1964 cent in a TPG holder which is questionable, if you compare the two the three strikes on the cent are very crisp with little metal flow at the edge of the O/C strikes. On this nickel you can see definite metal flow and severe distortion of the underlying strikes consistent with the high pressures of a genuine strike.
