The use of similar 'stock' photos seemed pervasive back then:
On the rarity of this card - I think the idea that the card was a 'prototype' or proof for a future set as part of a game is more than likely - as may have been the case with any (or none) of the nearly 1/1 types cards/sets that are out there.
Since this is Ruth, I would think the competition for the card may be entertaining!

I personally like the sleeves. To many dented edges from loose fitting holders out there.

It probably would've if not for the tear halfway down the right-hand side of the card.

Its a 1921 E-Unc. Shotwell Confectionery card, related to E121 or W575-1A, or as many call them the "E121 group"
It is from a prototype set that was never issued as planed, similar to the Herpolsheimer cards of the same year. Even the design is like the Herp cards from the same year (Herp cards are also unique ie a single known card for each player, no dups)
