My first ever submission to PSA went flawless. Ever since then, I have gotten this on every single sub since:

Why can't I just get one right the first time?

I got an email the other day, 2 months after my submission was logged, saying "there's actually 493 cards, not 492". It's been on receive hold for several days even though I gave 'em the ok to run the credit card for the extra card.....that'll teach me to let my 8 month old count out my cards
