How to submit coins for TruView

What's the procedure for a submission of TruView pictures? Just mark "other" on the form, write "TruView", and then add $15 per coin? Or do I check "Reholder" and "Other" (with TruView) and multiply the # of coins by the $10 (Reholder) service level fee, then multiply the # of coins by the $5 (TruView) Additional services fee, to arrive at $15 per coin charge? Thanks in advance. I have some coins I'd love to get photographed with this service that could stand the reholdering as well!
