(2) I've answered this type of question , a couple of times. My vote is not for the ugliest player, but one of nastiest looking cards. Its the 1961 Steve Bilko-- card 184--- He's plenty scary looking.
If I had a higher IQ , I'd post it myself. But sense reality is what reality is , if someone goes to PSA Set Registry area, clicks on teams, clicks on the 1961 Los Angeles Angels, the evil looking troll can be found and maybe posted. My thanks in advance.
If we were able to look at players hearts, Barry Bonds is the UGLIEST, possible in history, IMO.

Another Brick in the wall - Pascual is in the ugly HOF. Through '89 he had a career ERA of 3.48 in 1150+ innings and a 64-62 record. If he hadn't played for th ePirates, Braves and Expos in the 1980's we might have a different opinion of Pascual.
