How does PCGS assign new coin numbers?

Specifically, does the coin assigned to the "0" spot in the pop report rotate out? I noticed that a newly CAM'd pattern is in the spot right now. Has this been used as a temp spot in the past?

Also, can anyone link me to a guide that would explain how new numbers are created? i.e. If an off metal example of coin #2222 is found it might be designated #702222 -- are those rules listed somewhere?
