I'm curious myself. Can any expert answer how that one got out? I thought the coins were counted as they were bagged. Maybe they go on weight? I'd really like to know if anyone has an answer.

1999 was the last year before the mint clamped down on quality control. The armored car services would roll all the coins from the original mint bags for distribution. The coin counters would jam with most misformed coin errors. In my locale the armored car services employees could keep the error coins as long as they replaced it with their own pocket change. These employees would come to my dealer and sell these coins bulk.
The mint finally clamped down and told all armored car services that if they didn't turn in ALL coin errors they found,the US mint would fire them.
