It's a particular obverse die for the 1967 SMS Kennedy that has a birthmark shaped shiney frost break in his hair below the part. You can see it better in this image:
The die is known for producing some spectacular cameo examples.

See that thing on the side of Kennedy's head? You're thinking it's an ear, right? That's what most people assume, but Russ has a different idea. It's harmless, and he's happy.
