Stains on MS American Silver Eagles

MS American Silver Eagles seem to often come with white stains/spots.
Can anyone explain why this happens? Is it the way the planchets were rinsed? If so, how long does it take for the stains to appear?
How bad can the stains be before the coins don't grade MS69?
Do the stains get progressively worse over time?
