Is this the official entry form into the

What if my total invoice is $15 - will they send me a coin and $5?

That's a good question. The eMail doesn't mention anything about a minimum purchase.

I'm pretty sure it's for anybody who's signed up at Teletrade. I don't buy anything high dollar, so I doubt I'm on any special list.
